Everything went haywire last night when Claude Code CLI AND Claude Code API crashed… I had a TON of work in flight, and then agents started failing and crashing, and then work staging started bumping into each other (instructions forgotten/ignored, cross-thread nonsense, context warnings, documentation being wiped by subsequent agent cleanup actions that actually don't fail while everything else in the path did)
